Terms and Conditions

By booking or attending an appointment with Rise Physio+, you agree to the following Terms and Conditions.

1. Booking and Consent

By booking an appointment, you confirm that:

  • You are voluntarily requesting and consenting to the assessment, advice, treatment, therapy, training, rehabilitation, wellness, or other service associated with the appointment selected.
  • You have selected the appointment type that best matches the service you wish to receive.
  • You will provide accurate and relevant information regarding your health, medical history, and any circumstances that may affect the service provided.
  • You understand that you may ask questions about any aspect of the service and may withdraw your consent at any time.

     

2. Recovery and Outcomes

Our team is committed to helping you achieve the best possible outcome and supporting your recovery and wellbeing goals.

However, every individual and condition is unique. The response to services, rate of recovery, and overall outcomes may vary from person to person and cannot be guaranteed.

 

3. Appointment Attendance

Clients are expected to arrive on time for their scheduled appointment.

If you arrive late, your appointment may need to be shortened in order to avoid delays for other clients. The full appointment fee will still apply.

 

4. Cancellation and Missed Appointment Policy

We require a minimum of 24 hours’ notice for any appointment cancellation or rescheduling.

Appointments cancelled with less than 24 hours’ notice, or appointments that are not attended, will be charged at 100% of the appointment fee.

 

5. Non-Attendance

Clients who fail to attend an appointment without providing notice will be charged the full appointment fee.

Following a “No-Show”, any future appointments may only be booked on a prepaid basis. Rise Physio+ reserves the right to require payment in advance before confirming subsequent bookings.

 

6. Minors

Clients under the age of 18 must be accompanied by a parent, legal guardian, or responsible adult for the duration of their appointment unless otherwise agreed by the clinician.

The accompanying adult may be required to provide consent on behalf of the minor where appropriate.

7. Fees and Payment

All fees are payable at the time of the appointment unless otherwise agreed in writing.

Rise Physio+ reserves the right to amend fees at any time. Current fees will be displayed or communicated at the time of booking.

8. Privacy and Data Protection

Personal and health information collected by Rise Physio+ will be processed in accordance with our Privacy Policy and applicable data protection legislation, including the General Data Protection Regulation (GDPR).

9. Right to Refuse Service

Rise Physio+ reserves the right to decline, discontinue, or reschedule services where it is deemed clinically appropriate, where client behaviour is inappropriate, or where these Terms and Conditions are not adhered to.

10. Acceptance of Terms

By booking, attending, or receiving services from Rise Physio+, you acknowledge that you have read, understood, and agree to these Terms and Conditions.
